How to get an invite to create an account, create a password and provide consent to share your details with your provider

1.1 Invitation notification

  1. If you are invited to a pathway by your healthcare provider, you will receive a direct notification by SMS or email.

  2. This notification will contain a message from your healthcare provider and a unique link. This link will allow you to create an account if this is your first time.

  3. You can open the link from your mobile device or copy this link into the web browser of your tablet or computer.

1.2 The invitation process

  1. Once you follow your unique link you will be asked to choose whether the invitation is for yourself, or you are acting on behalf of another person.

  2. Create a password and repeat your password in the second box. If you already have an account with Personify Care, enter your password or reset your password via the "Reset password" link at the bottom of the page.

  3. Once you have accepted the Terms of Service and Privacy Policy press "Register" to create an account. If you already have an account, you can skip to Provide consent below.

Note: If you have chosen email for communication, you will also be asked to enter a 5-digit code that will be sent to you for security purposes. If you have difficulty receiving the code, please contact your health provider.

1.3 Enter your details

Invitation is for yourself

On the “Patient Details” screen, you will be asked to enter your personal details. These details are required to identify you in Personify Care.

Note: You will not be able to modify this information once it is submitted. If you need to change your personal details later, please contact your healthcare provider to request changes.

Invitation is on behalf of someone

If you are registering on behalf of someone, we will ask for your details and theirs.

You will be creating an account for yourself and an account for the other person under the one mobile number. This will allow your healthcare provider to identify who has filled in the information and for whom.

Please enter your details first, and then the other person’s details.

Note: The system allows for the same mobile number to be used for multiple accounts.

1.4 Provide consent

The last step before you can begin a pathway is to consent to sharing your information and progress with your clinical team. Do this by selecting “I accept”.

Note: This consent process will need to be completed again for each healthcare provider and each person that you are acting on behalf of.
