How do I register as a proxy on behalf of someone?

Need to fill out someone's activities or do you need someone else to fill out yours?

There are times when either yourself or another may need someone else to fill out their forms and complete activities.

Some common scenarios where a proxy registration can be used include:

  • A parent that is filling out the forms for their child

  • A family member or friend is filling out the forms on behalf of someone who is unable to use a device.

  • Multiple family members share a single mobile phone number

Note: if you as the proxy didn't receive the registration message, please contact the hospital to have them send an invitation registration message to you by telling them your mobile number or email address.

Once you are on the registration page, you should be presented with a screen similar to the following:

First, at the top of the page, select On behalf of someone else for the Who are you registering for today? question.

After that, enter in a chosen password into both of the password fields, check the box for the I accept the Terms of Service and Privacy Policy field and then click on the Register button.

You should then be presented with a screen similar to the following:

To complete this form enter in your own personal details in the ENTER YOUR DETAILS section i.e. Your first name, Your last name, Your date of birth and the details of the person you will be acting on behalf of in the ENTER PATIENT DETAILS section. i.e. Patient first name, Patient last name, Patient date of birth.

For example, if a parent (John) was wanting to be the proxy on behalf of their daughter (Jane) they might fill out the form as shown:

Once you have completed filling out fields, click on Next to continue. At this stage, you will be completing everything for the person who you are acting for.

At the top of screen you should now see a message at the top of every page such as the following:

In cases where both parties have accounts, which would be the case if two people are sharing a mobile phone number you will be able to switch between the pathways for the two patients using a screen like the one below. The top pathway is for the first account (in this example, the account of John Doe), the second pathway belongs to Jane Doe.

Clicking on the the top pathway results in a screen like below, which shows the outstanding activities that John needs to complete. The message in the top area of the screen allows switching between the multiple pathways.

Clicking on Jane's pathway results in something like the screen shown below, in this case showing that Jane has completed all of her activities for today and there is nothing further she needs to do at this point in time.
