Steps
Open your invitation
When your healthcare provider adds you to a care pathway, you will receive an invitation by SMS or email containing a message from your provider and a unique link.
Tap the link on your mobile phone, or copy it into a browser on your tablet or computerChoose who the account is for
Once you open the link, you will be asked whether you are registering:
For yourself, or
On behalf of someone else (for example, a child or family member)
If you have an existing account and have been invited specifically to act on behalf of a new person you will see this. We suggest following our “How do I register on behalf of someone else” steps for further information. If you are sharing a mobile number with someone else, and that person already has an account, also select the second option.
Create a password
Enter a password and re-enter it to confirm. Your password must be at least 8 characters.
If you already have a Personify Care account (or you are sharing a mobile number with another person), enter the existing password. If you don’t know the password, follow our Password reset steps here.
Read the Terms of Service and Privacy Policy, then tap Register.
You may also be asked to enter a 5-digit code that will be sent to you for security purposes. If you have difficulty receiving the code, please contact your health provider.Enter your details
If you are registering for yourself:
Enter your personal details on the Patient Details screen. These are used to identify you in Personify Care.
If you are registering on behalf of someone else:
You will be asked to enter your own details first, then the other person's details. Note that if you have registered before, the Enter your details section may not appear. This creates two linked accounts under the same mobile number, which allows your healthcare provider to identify both who filled in the information and who it is for. We suggest following our “How do I register on behalf of someone else” steps for further information.
Please note: You will not be able to change these personal details yourself after submitting. If a correction is needed later, contact your healthcare provider - see our Problem with account information article.
Provide consent
The final step is to consent to sharing your information and progress with your clinical team. Tap "I accept" to proceed.
You will need to complete this consent step separately for each healthcare provider and for each person you are registering on behalf of.
You are now set up and ready to begin your care pathway.
Frequently asked questions
I already have a Personify Care account from a previous provider. Do I need to create a new one?
No - enter your existing password when prompted. Your new care pathway will be added to your existing account.
I made a mistake entering my details during registration
You cannot change your own details after submitting. Contact your healthcare provider to request a correction - see Problem with account information.
The verification code for email isn't arriving
Check your SPAM folder. If it's still not there, contact your healthcare provider.
