Why can't I change my own details?
To keep your account secure, certain details cannot be changed by users directly. The following information can only be updated by your clinical team:
First and last name
Date of birth
Email address
Mobile number
Because these details are used to verify your identity and may also be recorded in their system, your clinical team needs to confirm who you are before making changes.
How to request a change
If your account has Messaging enabled
Send a message to your clinical team directly through the app. Let them know which detail needs to be corrected and they will update it for you.
If Messaging is not available in your account:
Contact your clinical team directly by phone. They can access your Personify Care account and make the change on your behalf.
Frequently asked questions
My date of birth is wrong and I can't log in.
If an incorrect date of birth was entered when your account was created, you will not be able to log in using that method. Contact your clinical team or reach out to us via the chat button below - our support team can help arrange a correction.
I don't know how to reach my clinical team.
