Having issue with incorrect or changed date of birth, first or last name, email address or mobile number.

To keep your information secured after your account creation, it is not possible to edit your patient account details such as:

  • First and Last name

  • Date of birth

  • Email

These details can only be changed by your clinical team. They will need to verify your identity and access your Personify Care account to change these important identification details.

If the Messaging feature is available in your patient account, you can use it to communicate with your clinical team directly (see screenshot below). Otherwise, please contact your clinical team directly via telephone.
