For returning patients - your password is not working or if you have forgotten your password

Some accounts also allow you to log in using your date of birth. If you try to log in this way, make sure you have your mobile phone with you as it will be needed to receive a confirmation code from us.

  • Accounts that allow date of birth logins will show you a page like this:

  • Click on the link

  • Enter your mobile phone number (without spaces, commas or dashes) into the Mobile number or Username field.

  • Then enter your date of birth and click Submit. At this point, we will automatically send a confirmation code to your mobile phone via SMS - enter that confirmation code into the Code field and click Submit.

Note: Wait at least a minute for the code to be sent before requesting a new one.
